NOTE: Form must be turned into show office with office fee PRIOR to start of race Friday. (Or first day of event).
This form will not be accepted by fax, email, etc. New rider must pay office fee at show office. In the event the new rider cannot submit form in person by start of race, new rider may notify Renea by text PRIOR to start of race 918-617-0660, and we will allow you to turn form in upon arrival. However, changes will NOT be made until form is turned into show office and office fees are paid.
TO COLLECT WINNINGS
We cannot mail winnings until we have a signed w9 form (FOR THE CURRENT YEAR) on file with your address and social security number AND signed by you. Note we will NO LONGER accept a picture (or jpg) of the form. It must be a completed pdf form if you are emailing, otherwise you will need to mail a hard copy to our PO Box 306 Checotah OK 74426. THESE FORMS ARE ALWAYS AVAILABLE AT THE SHOW OFFICE. Thank you.
